How Tributes Work Step-by-Step
Every tribute story shared with Lifeline of Ohio is personal and meaningful. We encourage you to tell your story from the heart while being mindful not to include confidential information. Having a friend or family member proofread your story can be helpful. Above all, your tribute story honors the gift of life and should reflect hope, inspiration, and positivity.

1. Before You Start
You may want to prepare the following:
- A written reflection of your loved one’s life. This can include meaningful memories, what they loved, or words that best describe who they were.
- A profile picture of your loved one. All profile photos will be square-cropped upon upload.
- Additional photos of your loved one to be displayed in a gallery on the tribute page (max. 12 photos).
2. Start the Form to Add a Tribute
To begin creating your loved one’s tribute page, complete this form.
3. Submit for Review
Once you have completed the form, Lifeline of Ohio’s Aftercare Team will approve the submission prior to the tribute being posted.
4. Tribute is Posted
Please allow 3-4 weeks for the approval process. You will receive an email confirmation once your loved one’s tribute is posted.
If any changes to the tribute are needed after it is posted, use the “Request an Edit to This Tribute” button at the bottom of the tribute page.
Invite Family and Friends to Post Memories
Family members and friends can post messages, memories, and words of love on tribute pages. This creates a space to offer comfort, connection, and a reflection of the impact your loved one had on others.
Share Your Loved One's Tribute with Others
Tribute pages are meant to be shared. Lifeline of Ohio created Tributes of Hope to honor the lives of those who made donation possible. We invite you to share their tribute page with others on social media or by using the direct link to continue the ripple effect – how one generous act changes countless lives and creates endless possibilities.
